I think, that human rights shouldn't be paid for. Therefore, something that is a human right should be guaranteed by people who are elected to look after the society they run. So it should be at least mostly free, there's some reasons to consider both versions, but I won't accept a model as inaccessible as America's is, it's not only unfair, but that impacts productivity in the society. People are motivated by happiness, by feeling appreciated, it will help business. Especially seeing as it can save lives, those people can't contribute when they're dead, or crippled for that matter.
