It started out that way, as a visual gag about what happens to virgin boys who get overstimulated by surprise female nudity. The rational being, when a guy is turned on, blood floods to a certain part of their anatomy, however a virgin's body might not have the experience to know where to send it, and is not prepared to cope with that much excitement.
Over the years, the restrictions have relaxed. Now they might use the gag for any person (male, female, old, young) who is suddenly surprised by something they find very sexually arousing. Generally not used for characters who have an active sex life and are thus accustomed to being turned on.
